TERMS AND CONDITIONS OF USE

MyOM is based on the “Opioid Manager™”, a tool for clinicians, and is a companion to the “Opioid Manager™” which is used by medical practitioners. MyOM is intended as a reference for patients currently using opioids for chronic non-cancer pain, or for patients with chronic non-cancer pain who have been offered opioids to help manage their pain. It is not a medical device and is not designed for use without the supervision of a trained medical practitioner. Nothing contained in MyOM should be construed, directly or indirectly, as the practice of medicine or providing medical services by Toronto Rehabilitation Institute,University Health Network.

MyOM is not a substitute for professional medical advice, diagnosis or treatment. Never make changes to your medication, nor adjust your dose, without first consulting your health care provider. Always seek the advice of a physician or other qualified healthcare provider concerning questions you have regarding a medical condition, and before starting, stopping or modifying any treatment or medication. Never delay obtaining medical advice or disregard medical advice because of something you have or have not read on MyOM. If you have, or suspect you have, a health problem, or if you experience an adverse side effect, please consult your doctor. If you have, or suspect you are experiencing a health emergency, please promptly visit a Hospital Emergency Department in your area.

MyOM has not been evaluated by Health Canada, the United States Food and Drug Administration (“USFDA”) or any other domestic or foreign regulatory agency and is not approved to diagnose, treat, cure or prevent any disease or for the prescription of any medication or treatment. While the information contained may be of use in other jurisdictions, these materials were designed for use in Canada and you are solely responsible for ensuring suitability and permitted use in other jurisdictions.

COLLECTION OF DATA

We collect non-personally identifiable data (which is transmitted back to us) through the App regarding the use of the App in order to better understand use of the App and to help us identify areas for improvement.

The App also allows users to enter certain identifying information and personal health information, which they can then choose to transmit directly to their health care provider by email. Any such information is not collected by us and no copy is sent to us by the App. Please keep in mind that information sent by email is not encrypted and may be subject to interception by third parties.

The Opioid Manager App is a project of Toronto Rehabilitation Institute, a member institution of the University Health Network.

The “Opioid Manager App” (the “App”) is based on the “Opioid Manager™”

The “Opioid Manager™” was developed by Toronto Rehab, the Centre for Effective Practice in Toronto (www.effectivepractice.org) and the National Opioid Use Guideline Group (NOUGG) as a point of care tool for providers prescribing opioids for chronic non-cancer pain. It condenses key elements from the Canadian Opioid Guideline (http://nationalpaincentre.mcmaster.ca/opioid/). The Opioid Manager is distributed electronically at the Guideline’s website (http://nationalpaincentre.mcmaster.ca/opioidmanager/)

For providers prescribing opioids for chronic non-cancer pain: The “Opioid Manager App” is intended to be used as a point of care tool for providers prescribing opioids for chronic non-cancer pain. It is not a substitute for professional medical advice, diagnosis or treatment. The application allows you to print and/or e-mail the information that was entered. After the Application is closed, all information that was entered is lost.

If you are NOT a provider prescribing opioids for chronic non-cancer pain: The application is not a substitute for professional medical advice, diagnosis or treatment. If you have or suspect you have a health problem, please consult your doctor. If you have or suspect you are experiencing a health emergency, please promptly visit a Hospital Emergency Department in your area. No personal identifier is entered in this application. The application allows you to print and/or e-mail the information that was entered. After the Application is closed, all information is lost. Medical staff do not access or read personal information you enter into the application, it does not become part of your medical records. If you have questions, concerns, or corrections regarding your information please contact a member of your health care team or discuss this with them at your next appointment.
